Required Forms

The following U.S. Coast Guard forms are required for licensing. Forms do not have to be complete prior to attending class. To read and print these forms, you must have Adobe Acrobat Reader.

• Drug Testing Form
• Drug Testing Instructions
• Small Vessel Sea Service Form

• Driver's License or photo ID (first day of class)
• Original birth certificate or current U.S. Passport
• First Aid and CPR certification within the past 12 months
          (official certification from the administering organization)
• Three (3) character references
• Sea Summary form (if you served on more than one vessel)
• Two (2) fingerprint cards (provided first day of class)
